MV Marten, Port of Brake - Collapse of a Grain Bulkhead During Shifting Operation caused 2 injured Seamens.

On 03 June 2026, prior to commencement of grain loading operations, a movable grain bulkhead was being shifted from its stowage position to Frame 62 within the cargo hold. According to vessel records, a safety briefing, risk assessment, and work permit were completed before the operation commenced. Communication between personnel was maintained via portable VHF radios.

After the bulkhead reached its designated position, the lower securing pins were installed. Several crew members reported to the Chief Officer that the upper securing pins had also been properly engaged and that the bulkhead was secured. Based on this information, the Chief Officer commenced movement of the hatch cover to position the next bulkhead.

It subsequently became evident that the upper securing pins had not been properly engaged. The bulkhead lost stability and collapsed into the cargo hold. Two crew members who were within the danger zone attempted to move clear of the falling structure, fell while escaping, and sustained injuries to their feet and lower limbs.


Conclusion

The investigation concludes that the accident was not caused by equipment failure, but rather by a combination of human error, inadequate verification processes, ineffective communication, and insufficient control of the hazard zone. The bulkhead was reported as secured although the upper securing pins had not been properly engaged. Continuation of the operation under these circumstances resulted in loss of stability and collapse of the bulkhead, causing injuries to two crew members.


Investigation Outcome:

The accident was preventable through effective verification, supervision, communication, and hazard zone management. The corrective and preventive actions identified in this report should be implemented throughout the fleet to prevent recurrence.
